Job summary

Advance Care Alliance New York is hiring Care Managers to support individuals with intellectual and developmental disabilities. You will coordinate services, advocate for members, and help create person-centered Life Plans that drive real impact.

The role involves field-based work with local travel, a hybrid in-office/field schedule, and collaboration with multidisciplinary teams to ensure comprehensive, high-quality care.

Qualifications

• Bachelor’s degree + 2 years’ experience OR 2 years caring for a family member with a disability.
• Master’s + 1 year, OR RN + 2 years, or equivalent relevant experience preferred.
• Experience in human services, I/DD, mental health, or related field preferred.

Responsibilities

• Develop and manage individualized Life Plans based on comprehensive assessments.
• Coordinate medical, behavioral health, and community-based services.
• Advocate for members to access services, benefits, and supports.
• Lead interdisciplinary team meetings and collaborate with providers.
• Monitor progress, address gaps, and ensure high-quality care delivery.
• Maintain accurate documentation and compliance with regulatory standards.
• Support transitions across care settings and life stages.
