Job description

Join to apply for the Care Manager role at Care Design New York.

As a Care Design New York Care Manager, you will help people with intellectual and/or developmental disabilities identify and realize their long-term and short-term goals by developing, implementing, and monitoring their person-centered Life Plans. These plans help our members live their best life — so we’ll count on your relational and organizational skills. All Care Managers receive comprehensive training, regardless of background. You will bring your passion for service into our care model to deliver the highest standards of service to our members.

This position is hybrid, involving visits with members and their families at home, attending trainings and meetings at the office, and working remotely. Support outside NYC requires a driver’s license, reliable transportation, and appropriate insurance.

Responsibilities include:
  1. Advocating for members to ensure informed decision-making and guardianship.
  2. Leading meetings and assessments to develop comprehensive, person-centered Life Plans.
  3. Implementing, updating, and monitoring Life Plans, facilitating reviews and approvals.
  4. Integrating supports and services across medical, behavioral, social, and community domains.
  5. Maintaining contact with key individuals in members’ lives.
  6. Submitting documentation timely, including Life Plans and progress notes.
  7. Assisting members with benefits management, such as Social Security, Medicaid, Medicare, and Food Stamps.
  8. Helping resolve living problems related to housing, utilities, safety, and legal issues.
  9. Reporting abuse or neglect immediately.
Qualifications:
  • Bachelor’s degree with 2+ years relevant experience, OR
  • Registered Nurse license with 2+ years relevant experience, OR
  • Master’s degree with 1+ year relevant experience.
  • Strong communication, interpersonal, and organizational skills are required.

Salary: $27.75/hour, with potential increase to $30.19/hour.
