Center for Primary Care is hiring a Care Coordinator to support chronic care management across our Augusta, GA region. The role involves patient outreach, monitoring health status, reviewing conditions, ensuring adherence to care plans, and coordinating seamless care with medical staff.
The position is remote/hybrid with a typical full-time schedule. Requires high school diploma, 1–2 years of medical assistant experience, and licensure/certification as MA or LPN is preferred.

The Care Coordinator works under the direct supervision of the Manager of Chronic Care with responsibility to the Director of Operations. The Coordinator conducts patient outreach and is responsible for monitoring patient health status, review patients' conditions and symptoms, evaluation of patients' compliance with physician directed care, and orchestrating seamless care for patients.

Education: High School graduate or GED required.
Licensure/Certification: Registered MA, Certified MA, or current LPN required. Basic Life Support or be able to obtain within 6 months.
Experience: 1-2 years of Medical Assistant or similar medical experience.
Work Setting: Remote/hybrid. This position may require limited travel throughout the business day.
Job Type: Full-Time
Schedule: Weekdays and a bility to work flexible and extended hours, as needed.
Compensation: Market competitive base pay, commensurate with education and experience.
