TheKey is seeking a Geriatric Care Manager to provide holistic, client-centered care for aging adults at home. The role covers assessments, reassessments, and managing medical and cognitive challenges while honoring the client’s dignity, independence, and wishes.
Responsibilities include developing comprehensive care plans, tracking client status, coordinating with families, and managing electronic health records and billing.

As a Geriatric Care Manager, you are responsible to provide a holistic, client-centered approach to caring for clients. The Geriatric Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.
