Job summary

TheKey is seeking a Part-Time Geriatric Care Manager to provide holistic, client-centered care. You will assess and reassess needs, address medical and cognitive challenges, and ensure clients’ dignity and independence are respected.

You will collaborate with families, physicians, hospice, and social supports to develop and manage comprehensive care plans, including documentation and resource coordination. Flexible work options may be available.

Qualifications

• Minimum 5 years in community, hospital, hospice or home health case management.
• Bachelor's or master's in a healthcare-related field; advanced certification preferred.
• CMC or CCM certification is required or preferred.

Responsibilities

• Develop and implement comprehensive care plans for clients.
• Track and document observations, changes in preferences, and medical orders.
• Monitor client status and outcomes; coordinate with families and professionals.
• Guide families to make informed medical decisions and manage referrals.
• Maintain electronic health records and assist with billing as needed.

The Role

As a Part-Time Geriatric Care Manager, you are responsible to provide a holistic, client-centered approach to caring for clients. The Geriatric Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.

Essential Duties and Responsibilities

• Development of Comprehensive Care Plans and implementation of Care Plans
• Track, assess and document observations regarding client condition, changes in preferences and problems/opportunities, medical orders, family requests and other information pertinent to Care Plan.
• Monitor, Evaluate and report on Client Status, Actions/Interventions, and Outcomes
• Works with formal and informal social support systems and families to ensure client’s needs are met.
• Provide guidance, resources, and information to enable families and clients to make informed medical decisions.
• Manage Electronic Health records for clients including billing.
• Coordinate resources for client care.
• Interact with other professionals on the client’s behalf this may include, physicians, attorneys, fiduciaries, hospice personnel, and community personnel.
• Create and maintain referral resources, identification, coordination, management and advocacy
• Additional duties and responsibilities as may be assigned by your supervisor

Required Skills, Education and Certifications

• (5) Years’ experience in community, hospital, hospice or home health case management
• Bachelor’s or master’s degree in Healthcare Related Field, preferred
• Certified Care Manager (CMC) or Certified Case Manager (CCM)
• Demonstrated knowledge of aging, dementia, and/or disabilities.
• Experience working with complicated family systems and dynamics.
• Computer Skills: MS Office, Electronic Health Records and Internet Skills
• Excellent customer service and problem- solving skills
• Excellent verbal and writing skills
• Detail-oriented with the ability to keep up with fast paced processes

Physical Requirements

• Ability to lift and carry up to 10-15 pounds
• Ability to sit, stand and walk for prolonged periods of time throughout the work day
• Frequent local travel by personal automobile to client homes required
• Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
