Job summary

Community Care is hiring a Case Manager/Care Manager in Milwaukee, WI, Monday–Friday, 8:00 a.m. to 4:30 p.m., with an $8,000 sign-on bonus. This role focuses on family care program case management, collaboration, and helping adults with disabilities live independently in the community.

The position requires a Bachelor's in Human Services (or related field); relevant licensure is preferred and a minimum of 3 years experience with older adults or disabilities is expected. Travel is required.

Qualifications

• Bachelor's Degree in Human Services or closely related field required.
• Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required).
• Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities; 3 years of community nursing preferred.

Responsibilities

• Manage caseload of Family Care Program members.
• Complete comprehensive clinical assessments.
• Develop, member-centered care plans, provide ongoing care coordination, member education, quality assurance and monitoring of services.
• Travel locally using personal vehicle, valid driver's license and proof of required insurance.

Case Manager/Care Manager Job Responsibilities:

• Manage caseload of Family Care Program members
• Complete comprehensive clinical assessments
• Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
• Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
