Job summary

Jobtailor is hiring a qualified Care Manager to implement individualized care plans for clients in New Hampshire. You will conduct home visits, coordinate with families and care teams, and ensure high-quality elder and disability care.

The role requires a Bachelor’s degree (social work or related field), LSW/LISW preferred, and at least 1 year of relevant experience. You’ll travel within your region and maintain accurate clinical records while complying with NH Medicaid regulations.

Qualifications

  • Bachelor’s degree required; preferably in social work, human services, or related field.
  • LSW or LISW in state of employment preferred.
  • At least one (1) year of experience working with elders or adults with disabilities.
  • Ability to travel within geographic area; valid driver’s license and automobile insurance required.
  • Experience in case management, care planning and home care settings.
  • Training, evaluating, and supervising caregivers.
  • Documenting accurately; maintain a clinical record for each patient.
  • Proficiency in MS Office and case management systems; EVV knowledge.

Job description

  • Facilitate smooth transitions and ongoing care for clients.
  • Maintain compliance with all relevant regulations and standards.
Requirements

  • Ability to travel (by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required.
  • Experience in case management, care implementation, planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting.
  • Training, evaluating, and supervising caregivers.
  • Documenting in an accurate and timely manner; maintain a clinical record for each patient.
  • Proficiency in common software and web-based applications (e.g. MS Office, case management systems, EVV, etc.).
  • Patient-centered approach with strong communication skills.
  • Familiarity with New Hampshire Medicaid, CFI and/or caregiving ecosystem (including CMAs, government offices, and other stakeholders) is preferred.
