Axelon Services Corporation is seeking a licensed behavioral health professional or RN to coordinate services for foster care members. The role requires 1 year of cross-system coordination experience, strong communication, and adaptability.

Travel is extensive within Central KS; duties include 48-hour visits after placement changes, 7-day team meetings, and ongoing assessments. Position runs 6 months with potential extension, typical 8am–5pm hours with occasional evenings.

Qualifications

• Master's degree in Behavioral Health, Social Work, or an accredited nursing degree.
• Licensed master’s behavioral health professional or RN as required by state contract.
• Minimum 1 year coordinating services across child welfare and related systems.

Responsabilités

• Support members in foster care and coordinate services to meet their needs.

• Conduct telephonic outreaches based on member acuity.
• Perform in-person visits based on member acuity.
• Complete annual assessments.

• Track completion and timeliness of Health Risk Assessment and Plan of Service.

Summary

Location: 75% Travel to members' homes, ideally located in Central KS - Salina, Hutchinson, Great Bend, Pratt, and surrounding areas

Duration: 6 months, possible extension

Shift: 8am-5pm, with possibility of some evening hours due to business need

Responsibilities

• Support members in foster care and coordinate services to ensure their needs are met
• Conduct 48-hour in-person visits following a placement change for the member
• Participate in 7-day team meetings following a placement change for the member
• Conduct telephonic outreaches based on member’s acuity
• Perform in-person visits based on member’s acuity
• Complete annual assessments
• Attend in-person visits that may take place after 5pm
• Participate in weekly team meetings
• Track completion and timeliness of Health Risk Assessment and Plan of Service

Requirements

• Master's degree in Behavioral Health, Social Work, or a degree from an Accredited School of Nursing
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience
• Minimum of 1 year of experience coordinating services across child welfare and related systems of care
• Strong communication, high-quality documentation, adaptability/flexibility, and solution-focused approach
