Job description

Title: Care Manager

Location: Remote, Region 4- Nassau, Volusia, Clay, Flagler, St. Johns, Baker, Duval

Duration: 3 Months (with potential for extension and future conversion based on plan needs, confirmation from Senior Leadership and HRBP)

Shift: 8-5 M-F, no OT

Responsibilities:
  1. Manage a caseload of approximately 150 members, primarily Tier 3 and Tier 4 behavioral health cases, acting as a subject matter expert within the region to support and educate colleagues.
  2. Address the increasing needs of members with complex behavioral health issues by providing specialized support to members, families, and team members.
  3. Perform day-to-day operations as a BH Case Manager, maintaining the same expectations as a full-time employee, reporting to the BH Supervisor and BH Manager, and participating in regional team meetings.
  4. Coordinate care, collaborate with providers and stakeholders, and document interactions accurately in the case management platform.
  5. Work independently within a supportive team environment, emphasizing ethical decision-making and best practices.
Education/Certification:
Required:Social Work, Psychology, Mental Health, Counselor Education
Preferred:Social Work, Psychology, Mental Health, Counselor Education
Licensure:
Required:LCSW or LMHC
Preferred:LCSW or LMHC
Must Haves:

At least 5 years of experience working with children and families within the system of care, and proficient in Microsoft Office and other platforms.

Nice to Haves:

Qualified Supervisors, Clinical Supervisor experience, familiarity with DCF, DJJ, APD, DOH, and/or CBC or ME.
