Job description

Immediate need for a talented Care Management Support Coordinator. This is a 5+ months contract opportunity with long-term potential and is located remotely in the U.S. Please review the job description below and contact me ASAP if you are interested.

Pay Range: $20 - $22 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

1. Shift Schedule: 8:00 AM to 5:00 PM PST, Monday to Friday
2. Assign at least 30 members daily for outreach. Make one call attempt per day to the numbers listed for each member.
3. If a number is invalid, PC will attempt to obtain a valid number.
4. If the member is reached, review CareGaps with them, assist with scheduling appointments, complete HRA if needed, and confirm they have no other questions or needs.
5. Performance Metrics include:

• 25 new members per day, 125 per week, and 500 per month
• Audit score of 90% or better
• Complete all outreach attempts within the 10-day maximum timeframe

Key Requirements and Technology Experience:

• Minimum 2 years of healthcare experience
• At least 1.75 years of experience with HIPAA procedures
• High volume inbound/outbound call experience
• High School Diploma required
