Job summary

Astrana Health is seeking a Supervisor, Care Management (Model of Care) in Monterey Park, CA. The role leads daily supervision of care management teams, ensures regulatory compliance, and coordinates cross-disciplinary efforts to deliver high-quality, patient-centered care.

You will oversee policy adherence, audits, MDT meetings, and collaboration with PCPs while supporting staff development and performance evaluations. The position offers hybrid work with a listed compensation range.

Qualifications

• Active RN or LVN unrestricted license required.
• Minimum of 3+ years of care management experience in a medical group, IPA, or HMO setting.
• Associate Degree of Science in Nursing (ASN) required.

Responsibilities

• Provide daily supervision of care management teams, ensuring adherence to ambulatory care management and model of care policies.
• Oversee clinical care management services across the continuum and support staff performance.
• Ensure compliance with NCQA standards, CMS, health plan regulations, and internal protocols.
• Lead MDT meetings and coordinate with network PCPs, specialists and vendors.
• Conduct performance evaluations and coaching for care management staff, and oversee hiring and onboarding.
