Job summary

MJHS seeks a Care Management Supervisor in New York, NY to oversee member care across the continuum, coordinating with PCPs, SCPs and the health care team to keep members independent and well served.

You will supervise assessments, ensure thorough documentation, and manage both direct and indirect reports while driving utilization and case management initiatives; a Master of Social Work (or RN/CSW) and 1–3 years of managerial experience are required.

Qualifications

• Master of Social Work Degree or related field, or RN or CSW.
• 1–3 years of management experience preferred.
• Experience in managed care required (utilization/case management).
• Current NYS CSW if CSW qualified; CCM preferred.

Oversees the management of member care with members, their families or significant others and coordinates with the PCP, SCP and all of the health care team facilitating the member's care across the continuum of care. The Care Management Supervisor also acts as a resource to staff reviewing cases, and makes recommendations for change, and supervises assessments of members' needs though coordination with the care team. In addition, supervises a wholistic variety of member's needs through communication with the health care team. The CMS also ensures a high level of complication with all documentation, provides efficacy reports, and manages the performance of their direct and indirect reports.

Requirements and qualifications:

• Master of Social Work Degree or related field, or RN or CSW.
• Minimum one to three years previous management experience preferred.
• Previous managed care experience in the areas of utilization management and/or case management required.
• Current NYS CSW if CSW qualified.
• CCM preferred.
