Job summary

Northwell Health is seeking a Supervisor of Care Management to guide clinical and non-clinical staff in managing at-risk patients. You will oversee day-to-day care managers, coordinate resources and support services across clinical, behavioral, acute and social care domains.

You will collaborate with internal teams on initiatives, ensure access and assessments, and train staff while upholding policies and high standards of care.

Qualifications

• Master's degree is required or equivalent combination of education and related experience.
• Current License to practice as a Master Social Worker in New York State is required, plus specialized certifications as needed.
• Current License to practice as a Clinical Social Worker in New York State is preferred.
• 4-6 years of relevant experience, and 0-2 years of leadership/management experience required.

Responsibilities

• Facilitate ongoing communication between care management staff and providers.
• Assign cases and oversee caseloads including care plan approval and file reviews.
• Perform related duties as required per ADA.
