Job summary

The Jewish Board is seeking a Social Work Supervisor to oversee care coordination for children with chronic medical and behavioral health needs in New York City. You will supervise care management staff, oversee enrollment/discharge, and manage high-risk caseloads while aligning with program protocols and quality standards.

Advanced licensure in a behavioral health field and 3+ years of relevant experience are required, along with strong communication skills and willingness to work some evenings

Qualifications

• Licensure in a Behavioral Health or related field (LMSW, LCSW, LMHC, LMFT, LCAT).
• A valid driver's license and access to a vehicle (Staten Island);
• A valid NYS ID for all boroughs.
• Minimum of three years of related experience with the target population.
• Experience in interdisciplinary teams and care management/coordination in medical or behavioral health settings.
• Excellent written and verbal communication and customer service skills.
• Fluency in a second language (e.g., Spanish, Mandarin, or Russian) preferred.

Responsibilities

• Provides regular weekly supervision to care management staff in Staten Island and Harlem as needed.
• Administrative oversight to ensure compliance with standards.
• Monitors service provision, referrals, and census data.
• Manages billing processes and documentation timelines.
• Oversees outreach, enrollments, and discharge processes per protocols.
• Manages high-risk caseloads to ensure continuity of care.
• Conducts quality assurance activities and audits; addresses incidents promptly.
• Engages in community outreach and builds linkage with service providers.
• Participates in network/coalition meetings.
• Covers for the Director in their absence.
