The Care Management Team plays a vital role in coordinating the movement of all patients through their inpatient stay at Sheltering Arms Institute, while honoring the treatment goals and plan of each individual patient. This effective and attentive planning for life beyond inpatient rehabilitation is also critical for SAI to meet organizational goals associated with our Mission, Vision, and Values.

Care Management specialists work directly with patients, families, the rehab team, third party payers, and external agencies to assure the development of a coordinated rehabilitation program that will maximize patient outcomes and create a safe, sustainable discharge plan.
