Job summary

AdventHealth in Parker, CO seeks an experienced RN to join our discharge planning team. You will participate in multidisciplinary rounds, educate patients and families, and coordinate post-acute care for timely transitions.

Ideal candidates have 2+ years hospital nursing experience and knowledge of discharge planning, CMS requirements, and utilization management. We offer part-time scheduling, day shifts, and a supportive work environment.

Qualifications

• RN licensure and active state registration.
• Minimum 2+ years hospital nursing experience.
• Strong discharge planning and care coordination knowledge.
• Experience with utilization management is preferred.
• Excellent communication with patients, families, and payors.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge needs.
• Educates patients and families on illness impacts and resources.
• Assesses readmitted patients and plans care transitions.
• Facilitates family care conferences with the team.
• Documents discharge plans and barriers to progression.
• Coordinates with payors for post-acute care authorization.
• Considers clinical and social factors for discharge planning.
• Reviews records and updates transition plans as needed.
• Develops contingency discharge plans to ensure timely care.
• Uses standard work to ensure complete medical records and reconciliation.
• Performs other duties as assigned.
