Job summary

Jewish Family And Children's Service of Greater Philadelphia is seeking a Program Manager for the Care Management Program to oversee a team of care managers, develop department goals, and ensure service quality across the Greater Philadelphia area, including remote work.

The role combines supervision with direct care management for a small caseload and requires travel across the region to client homes, offices, and telehealth settings.

Qualifications

• Master's degree from an accredited graduate school (MSW preferred).
• 2–4 years professional experience in social services or case management with supervisory experience.
• Extensive background in counseling and care management.
• Proficient use of technology (smartphone/tablet/laptop) and MS Office applications.
• Must pass background checks and meet vaccination requirements.

Responsibilities

• Coordinate and provide supportive counseling and care management services in client homes, telehealth, and offices.
• Conduct comprehensive assessments to develop mutually agreed care plans.
• Assist clients with resource navigation, budgeting, public entitlements.
• Collaborate with community resources and provide daily life skills coaching.
• Maintain accurate records including progress notes and billing.
• Lead team meetings and provide supervision and coaching.
