Job summary

Jewish Family and Children's Services seeks a Program Manager to oversee the Care Management program within the IFS Department. Qualifications

• Master's degree in social work or related field; MSW preferred.
• 2–4 years professional experience in social services or related fields; supervisory experience (2 years) preferred.
• Extensive background in counseling and care management; driving and travel as required.

Responsibilities

• Lead and supervise a care management team to deliver integrated services for IFS clients.
• Provide direct care to a small caseload, including home visits and telehealth.
• Coordinate assessments, resource navigation, and access to entitlements for clients.
• Develop and monitor program goals, budgets, and regulatory compliance.
• Foster community partnerships and participate in team and board meetings.
