Qualifications

• Minimum 5 years of relevant clinical experience in hospital, clinical, or home care settings.
• 3–5 years in case management, disease management, utilization review, or transition of care (CCM certification a plus).

Responsibilities

• Design and oversee care plans for patients on the panel.
• Serve as primary point of contact for patients and PCPs.
• Perform initial case management assessments and evaluate patient state across clinical and social dimensions.
• Develop multidisciplinary care plans and coordinate all elements of care plans.
• Maintain accurate records and ensure HIPAA compliance.
