Job summary

Together for Youth in New York state seeks a Care Management Program Coordinator to supervise 6-8 direct-reports and ensure services for youth and families meet TFY, HHSC and NYS-DOH standards.

Role includes on-call oversight, a small to moderate caseload when needed, and representing TFY's behavioral health services in the community and state, while partnering with diverse networks.

Qualifications

• Bachelor's degree in social work or related human services discipline required.
• Master's degree preferred.
• Minimum 3–5 years of experience with children and families.
• Minimum 2 years supervisory/management experience.
• Valid driver's license with clean driving record; ~20% regional travel.
• Flexible schedule including nights and weekends.

Responsibilities

• Supervise 6–8 direct-reports; ensure services align with TFY, HHSC & NYS-DOH policies.
• Participate in and initiate program development efforts.
• Collaborate with internal and external providers.
• Provide on-call administrative oversight on a rotating schedule.
• Carry a small to moderate caseload when necessary.
• Represent TFY's Behavioral Health services in the community and state.
• Serve on committees; liaise with schools, mental health programs, OCFS & managed care orgs.
• Conduct file reviews and ensure compliance with TFY, HHSC & DOH standards.
• Apply strength-based, person-centered approaches to service delivery.
• Supervise from a solution-focused perspective; work with Director to ensure program fidelity.
• Understand components affecting fiscal stability; monitor training for Care Managers.
• Perform other duties as assigned.

Job Responsibilities

• Care Management Program Coordinator supervises 6-8 direct-reports, Care Manager and is responsible for ensuring that Care Managers are providing services to youth and families in accordance with Together for Youth (TFY), HHSC & NYS-DOH policies.
• Participates in and initiates program development efforts.
• Collaborates with internal and external providers
• Provides on-call administrative oversight on a rotating schedule
• Carries a small to moderate caseload when necessary.
• Represents Together For Youth's Behavioral Health services in the community and throughout state.
• Serves on various agency and/or external committees; acts as liaison to community networks including schools, mental health programs, OCFS and Managed Care Organizations.
• Conducts file reviews, monitors charts, and maintains compliance as per TFY, HHSC and DOH standards.
• Demonstrated understanding and application of strength-based, person-centered approach to providing services
• Demonstrated ability to supervise from a solution-focused perspective
• Works in partnership with the Director of Care Management to ensure integrity and fidelity of the Care Management program.
• Ability to understand program components that affect fiscal stability
• Responsible for assigning and monitoring Care Manager training
• Perform other duties as assigned.

Job Requirements

• Minimum of Bachelor's Degree in social work or related human services discipline. Master's Degree preferred.
• Minimum of 3- 5 years of experience working with children and families required.
• Minimum of 2 years of supervisory/management experience required.
• Must have a valid driver's license and clean driving record. Regional Travel is required approximately 20% for the position. Must have access to a reliable vehicle with appropriate insurance.
• Must be able to work a flexible schedule including nights and weekends as needed.

Salary

• $60,000 - $64,500 annually; commensurate on experience
