This role reports to the Director and translates system plans into actionable department objectives while ensuring contract and policy compliance.

The MOCM fosters exemplary patient care, mentors staff, and supports professional nursing practice across the health system, with an emphasis on collaboration and continuous improvement.

Qualifications

• Bachelor's degree in Nursing required; Master's or Doctoral preferred.
• Texas nursing license or eligibility required; administration or clinical certification within 2 years.

Responsibilities

• Oversee operational, human resource, care coordination and transitional management of Outsourced Care Management.
• Translate system plans into departmental actions and monitor plan achievement.
• Ensure outsourced services align with policies, procedures and contracts.
• Provide clinical leadership to support professional nursing practice and teamwork.
• Mentor staff and participate in professional activities and community outreach.
