Job summary

Hartford HealthCare in Connecticut is seeking a Clinical Resource Management Nurse to join the Care Management team. The role focuses on timely care assessments at entry to ensure appropriate patient class based on clinical needs, working autonomously across system hospitals.

Responsibilities include applying criteria, coordinating with Emergency Department and Admitting Providers, educating groups, reviewing cases, and staying current with industry standards.

Qualifications

• Registered Nurse is required; BSN preferred.
• At least five (5) years of relevant clinical experience.
• Current State of Connecticut RN license.
• Familiarity with clinical care and reimbursement guidelines.
• Ability to influence, negotiate and make decisions. Strong communication skills.

Responsibilities

• Apply standard criteria to determine/validate patient class assigned by provider.
• Document review and findings and relay information to care team.
• Partner with Emergency Department/Admitting Provider to ensure appropriate patient class.
• Serve as resource and subject matter expert on patient class and level of care.
• Escalate cases when level of care is inappropriate.
• Maintain knowledge of industry standards and regulatory requirements.
• Provide education sessions to constituencies as needed.
• Support department needs during high demand or volume changes.
• Ensure critical information is relayed to the Care Manager.
• Complete assignments demonstrating expert criteria knowledge.
• Other duties as assigned.
