Job summary

Mercy Hospital Jefferson in Festus, MO is seeking a Care Management Associate to support the Care Management team with transition planning and length of stay coordination. Qualifications

• Education: High School Diploma, minimum required.
• Experience: 1 year Healthcare experience preferred; hospital experience preferred.
• Certification/Registration: BLS (CPR) at hire date or within 90 days of hire.
• Knowledge: Epic software preferred; strong interpersonal and organizational skills.
• Ability to work independently and with diverse groups; strong written and verbal communication.

Responsibilities

• Coordinate information and communication for the Care Management team.
• Support transition planning and length of stay management.
• Perform clerical tasks to assist Care Management workflow.
• Collaborate with healthcare team to ensure timely, accurate information for patients.
