Job summary

Emory Healthcare is seeking a Care Manager, Preceptor (RN or LMSW/LCSW) to train, mentor, and coach the Care Management Department. The CM Preceptor will cover roles as needed and manage an active caseload in care management and discharge planning.

The candidate will be a clinical expert, lead by example, and ensure policy adherence, training materials, and ongoing department education with audits and competency validations.

Qualifications

• Masters in Social Work or Bachelors in Nursing from an accredited institution.
• LMSW or LCSW license in Georgia required.
• 3–5 years healthcare experience; acute care setting preferred.
• Working knowledge of software/EEMR applications; ACM/CCM preferred.

Responsibilities

• Provide training, mentoring, and coaching for the Care Management Department.
• Onboard new hires and deliver ongoing departmental education and audits.
• Develop training plans and ensure materials are current and accessible.
• Collaborate with Care Management leadership to update policies and standard work.
