You will lead the Care Management team, partner with physicians and community resources, and drive discharge planning aligned with best practices.

Responsibilities include chart reviews for severity and service intensity, coordinating across departments, and guiding staff via performance

Qualifications

• Bachelor's Degree in nursing required or higher; RN licensure in applicable state.
• 3-5 years of Care Management experience required.
• 3-5 years of leadership experience preferred.

Responsibilities

• Oversee implementation and compliance of Care Management program.
• Lead staff, ensure accountability and proper scope of practice.
• Review charts for severity of illness and service intensity.
• Coordinate discharge planning and social services per regulations.
• Collaborate with physicians, staff and community resources.
• Lead performance evaluations and staff development.
• Maintain occupancy and regulatory readiness.
