Connect Health + Wellness is seeking a part-time Care Management Coordinator to support a patient-centered care model at our Ridgeway location. The CMA will assist the Quality Coordinator in identifying patients with complex issues, help with outreach, follow-up, and data collection in an integrated care setting.

The role emphasizes coordinating care, improving outcomes, and ensuring timely follow-up after hospital discharge, with a focus on quality and compliance in a Federally Qualified Health

Qualifications

  • Minimum one year experience in a physician office preferred
  • Prior experience in data entry required
  • A diverse set of skills and abilities including organization, prioritization, good grammar, telephone and interpersonal business manners and the ability to multitask
  • Good computer skills including knowledge of EMR systems (EMRs)
  • Experience with eClinicalWorks (eCW) EMR, preferred but not required
  • A passion and concern for the target population served by the organization

Responsibilities

  • Assist with identification of patients who have complex medical issues to aid in care coordination.
  • Assist in managing patient caseload and identifying patients that require additional assistance with barriers to care.
  • Ensure patients have appropriate follow-up after ED/Hospital discharge to decrease readmissions.
  • Ensure patients with missed appointments are rescheduled promptly to serve the patient population.
  • Reach out to patients with identified care gaps to meet value-based measures.
  • Participate in quality meetings to maintain patient-focused care and improved outcomes.
  • Participate in training opportunities to improve patient care, including cultural competency, effective communication, OSHA, HIPAA and EHR.

Care Management Coordinator (Quality) Position(part-time)

Connect Health + Wellness is seeking candidates for a part-time Care Management Coordinator for our organization.

Connect Health + Wellness is committed to providing primary health, dental and integrated behavioral health services at our two Federally Qualified Health Center sites and our Dental clinic by promoting health, reducing health risk factors, and increasing access to medical and dental services, primarily for the uninsured and underserved in the Martinsville-Henry County area. Additionally, we offer chronic disease self-management, medication assistance, and care coordination services to patients and local residents.

The Care Management Coordinator assists the Quality Coordinator by working with the Connect Health + Wellness staff to support a patient-centered model of care. Under direct supervision of the Quality Coordinator, the Care Management Coordinator assists with identification of patients with complex issues and/or those needing additional screening, as well as providing additional assistance, outreach, and follow-up. This person will work with other staff in a Patient Centered Medical Home environment the values teamwork and relentlessly pursues the highest levels of quality care for our patients.

Essential Functions of the Position

  • Assists with identification of patients who have complex medical issues (such as those having several diagnoses, chronic illnesses, complicated treatments, multiple providers) to aid in care coordination.
  • Assists in managing patient caseload and identifying patients that require additional assistance with barriers to care.
  • Ensures that patients have appropriate follow-up after ED/Hospital Discharge to decrease hospital re-admissions.
  • Ensures that patients with missed appointments are rescheduled in a timely manner to better serve our patient population.
  • Reaches out to patients with identified care gaps to meet required value-based measures.
  • Participates in quality meetings to maintain patient-focused care and improved outcomes for patients.
  • Participates in training(s) opportunities to improve patient care including, but not limited to, proactively addressing the needs of patients and families in this community, cultural competency, effective communication, OSHA, HIPAA and EHR.
  • Works under the guidance of the Quality Coordinator.
  • Assists Quality Coordinator with accurate data collection and reporting.
  • Performs these and other duties and responsibilities as assigned by the Quality Coordinator.

Pay Range: $15.00 to $18.00 per hour

Position Requirements

Minimum Required Education, Experience, Abilities & Characteristics

  • Minimum one year experience in a physician office preferred

  • Good computer skills including knowledge of Electronic Medical Record systems (EMRs)
    • Experience with eClinicalWorks (eCW) EMR, preferred but not required
  • A passion and concern for the target population served by the organization
  • Effective communication skills to work with the public
  • Licensure/Certification: Current Medical Assistant Certification through a recognized national professional association
  • AHA BLS-HCP Certification required
