Mercy is seeking a Care Manager Associate to support the Care Management team with transition planning and length of stay management in collaboration with an interdisciplinary team.

The role provides clerical support for the care management patient care model, ensures that department services align with patient needs, and assists with communications, prescriptions, and logistics to improve efficiency and patient satisfaction.

Qualifications

• High school diploma required.
• Minimum 1 year healthcare experience (hospital experience preferred).
• BLS (CPR) certification at hire date or within 90 days.

Responsibilities

• Support Care Management team with transition planning and length of stay management.
• Assist with prescription delivery/retrieval and transportation quotes.
• Communicate patient/family concerns to care managers and social workers.
