The Care Management Coordinator, Documentation Improvement actualizes the vision, mission, values, balanced scorecard performance measures for Montefiore Medical Center. She/he maintains standards of the practice for nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing’s Social Policy Statement. The Care Management Coordinator, Documentation Improvement [CMC-DI] is a Registered Professional Nurse with a sound base of current clinical expertise/knowledge and understanding of coding principles and guidelines. She/he reviews medical records to identify documentation improvement opportunities and then interacts with providers aiming for the provider to document the clarification within the body of the record. The CMC-DI is responsible for improving the overall quality and completeness of clinical documentation so it best reflects a patient’s severity of illness and risk of mortality and will therefore support appropriate reimbursement. Extensive interaction with providers at all levels is a large part of this position. Additionally, the CMC-D/I participates in performance improvement (PI) projects designed to continually improve documentation efforts at the medical center.
