Job description

Details: This position will work Monday through Friday from 8:00am until 4:30pm. No weekends or holidays! This team member must be able to work on-site at Methodist Hospital.

Bachelor's Degree or equivalent years of experience required.

Key responsibilities include partnering with Case Manager and our Social Work team daily. Assisting patients to their next level of care and answering any patient questions or concerns. Ensuring we meet all Medicare discharge requirements and arranging transportation for patients from hospital setting.

Consults with all necessary stakeholders to train and educate on various aspects of the Integrated Care Management process in order to achieve top quality results. Supports Integrated Care Management team to ensure appropriate discharge practices are carried out including but not limited to referral placement to home and extended care facilities. Complies with CMS guidelines by timely delivery of needed documentation. Assists in decreasing readmissions by ensuring post discharge primary care physician appointments are secure. Supports and monitors the outcomes of post-acute vendors with timely follow-up of patient concerns and monitors quality data in alignment of the IU Health Collaborative.

•Requires 0-3 years of related experience.
•Strong verbal and written communication skills are required.
•Advanced working knowledge of Microsoft Excel, Word and PowerPoint software applications is required.
•Requires excellent organizational and project management skills and ability.
•Must have strong analytical skills and be an accomplished problem-solver. Bachelor's Degree or equivalent years of experience required.
