Job summary

UChicago Medicine AdventHealth in La Grange, Illinois, is seeking an organized Care Coordinator to support patient access and care coordination. Schedule: Tuesday–Saturday, 8:00 a.m.–4:30 p.m.

This full-time day shift is based at 5101 Willow Springs Rd, La Grange, IL 60525. Responsibilities include promoting excellent customer service, coordinating with care managers, arranging transportation, obtaining insurance authorization, and documenting communications in patient records; requires HS

Qualifications

• High School diploma or equivalent required.
• 1+ office experience preferred, preferably in a medical setting.
• Data entry experience preferred.
• Strong computer and MS Office skills.

Responsibilities

• Promotes excellent customer service through interactions with patients, physicians, care managers, and the healthcare team.
• Collaborates with the Care Managers for patient care coordination needs.
• Coordinates patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.
• Provides and documents the delivery of regulatory letters to patients or their representatives in accordance with Federal and State regulations.
• Arranges patient transportation and follow up appointments as requested.
• Obtains insurance authorization for patient post-acute care needs as directed by the Care Manager.
• Other duties as assigned.

Education:

• High School Grad or Equiv [Required]

Work Experience:

• 1+ office experience [Preferred]
• 1+ office experience, preferably in a medical setting [Preferred]
• Previous data entry experience [Preferred]
