Job summary

100 LCMC Health seeks a Care Manager to be the hub of patient case management and care planning, coordinating with the care team to implement plans and transition strategies. You will develop individualized care plans, make outbound calls, and ensure quality across the continuum of care.

The role emphasizes collaboration with clinicians, education for patients and families, and efficient utilization of resources to achieve optimal outcomes and appropriate length of stay.

Qualifications

• Current nursing license to practice in Louisiana.
• 2 years of professional nursing or care management experience.

Responsibilities

• Develop individualized care plans, including prioritized goals, considering patient and caregiver goals and desired level of involvement.
• Make outbound calls to assess the patient’s current health status.
• Coordinate care for members and make referrals to outside sources.
• Ensure members discharging from hospital or ED receive necessary services and resources, including medication reconciliation.
• Triage symptom-based calls per approved protocols for a wide range of patients.
• Enroll patients and manage alerts for home monitoring programs.
• Provide approved general health information and guidance to callers.
• Document workflows in the EPIC system for tracking and quality assessment.
• Educate enrolled members on chronic and acute disease processes and self-management.
