Qualifications

• Requires MSW or health-related BA/BS degree.
• Preferred: three years in case management or social services.
• Preferred: one year in utilization review.
• Preferred: Certified Case Manager (CCM).

Responsibilities

• Collaborate with patient, family, medical staff, and agencies to determine appropriate care setting and hospitalization needs.
• Assist in discharge planning and utilization review in an acute care setting.
• Provide crisis intervention and psychosocial assessments to support patients and families.
• Implement interventions to ensure efficient clinical resource management and smooth transitions of care.
