Job summary

Northside Hospital is seeking a Care Coordinator to collaborate with neonate through geriatric patients, families and medical staff to determine appropriate care settings and hospitalization needs. You will participate in discharge planning and utilization review in a progressive acute care environment.

The role involves psychosocial assessment, crisis intervention, and ensuring efficient clinical resource management, with work hours from 7a–7p and no weekend or on-call requirements.

Qualifications

• Masters of Social Work (MSW) OR Bachelor's degree in health-related field.
• Three (3) years of experience in case management, social services or related field.
• One (1) year of experience in utilization review.
• Certified Case Manager.

Responsibilities

• Determine the appropriate setting of care and appropriateness of hospitalization.
• Assess post-hospital care needs and interventions to assure efficient clinical resource management.
• Involved in discharge planning and Utilization Review in a progressive acute care setting.
