TheETSUCollege of Nursing, Office of Practice invites applications for a full-time Care Coordinator (RN) for their Johnson City Community Health Center clinic.

The Care Coordinator (RN) partners with chronically ill or high-risk patients, their families, healthcare providers, and community resources to ensure timely access to care, increase preventative services, and reduce hospital readmissions and emergency visits. By developing personalized care plans and offering culturally appropriate education, the Care Coordinator empowers patients to manage their health more effectively and improves adherence to treatment.

This role involves daily one-on‑one patient meetings, coordinating follow‑ups with healthcare teams, and connecting patients to community resources. Success in this role leads to better patient outcomes, increased satisfaction, and reduced healthcare costs.

Knowledge, Skills, and Abilities
  • Evidence of essential leadership, communication, education, and counseling skills.
  • Proficiency in communication technologies (email, cell phone, etc.).
  • Highly organized with the ability to keep accurate notes and records.
  • Experience with health IT systems and reports is desirable.
  • Local knowledge about and connections to community health care and social welfare resources is desirable. The ability to speak a relevant second language is desirable.

  • Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal, and nonverbal.
  • Demonstrates a positive attitude and respectful, professional customer service.

Physical Demands

The employee must occasionally move (including lift, carry, push, and pull) up to 100 pounds or more. The employee may be required to move and/or stay in a stationary position in various settings for up to 7.5 hours at a time in addition to preparing and closing a clinical shift. Rapid mobility may also be required on occasion. Physical expectations of an assigned clinical facility may vary and be subject to change.

Stress levels may be high due to frequently dealing with numerous students, patients, family members, and other healthcare providers. This position demands concentration and the ability to work in emotionally charged situations.

Required Qualifications
  • Associate’s degree in Nursing.
  • Three years of relevant experience in clinical or community resource settings.
  • Active, unencumbered Registered Nurse licensure in the State of Tennessee or multi/compact state.
Preferred Qualifications
  • Bachelor’s degree in Nursing.
  • Experience in care coordination and/or case management.
