Job description

Responsibilities

The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. The role involves ensuring the patient's health needs are met while promoting quality, cost-effective outcomes, and adhering to hospital policies, standards of practice, and regulations. The focus is on care coordination, communication, and collaboration with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute service providers to achieve optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for patients and families by addressing patient choices, spiritual, cultural, language, and socioeconomic barriers to care transitions. Additionally, the role aims to enhance the patient experience.

Qualifications
  • Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing in lieu of 1 year experience.
  • California RN license
  • AHA BLS card
  • Ability to pass annual Inter-rater reliability tests for Utilization Review products used.
  • Ability to apply clinical guidelines to ensure progression of care.
  • Critical thinking and problem-solving skills.
  • Effective collaboration with multiple stakeholders.
  • Strong professional communication skills.
  • Understanding of how utilization management and case management programs integrate.
  • Ability to work as a team player and support team members.
  • Ability to thrive in a fast-paced, self-directed environment.
  • Knowledge of CMS standards and requirements.
  • Proficiency in prioritizing work and delegating tasks.
  • Excellent organizational and time management skills.
  • Strong customer service, interpersonal, and written communication skills.
  • Demonstrated analytical and problem-solving abilities.
  • Ability to manage multiple tasks or projects effectively.
Preferred Qualifications
  • Graduate of an accredited nursing school (Bachelor's Degree in Nursing - BSN) or related healthcare field.
  • At least five (5) years of nursing experience.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification preferred.
  • Knowledge of managed care and payer environments preferred.
