The Health Home Field Care Coordinator will serve as the primary case manager for high-cost, high-risk Medicare-Medicaid enrollees, focusing on intensive care coordination to improve health outcomes and reduce costs. Responsibilities include integrating care across multiple systems, reducing service gaps, and promoting patient activation and self-management for optimal health.

If located in King County, WA, you will have the flexibility to work remotely while tackling these challenges.

Primary Responsibilities
  1. Engage members, families, and caregivers face-to-face for comprehensive health needs assessments.
  2. Develop and implement person-centered Health Action Plans addressing medical, behavioral, social, and functional needs.
  3. Collaborate with care teams, providers, and community resources to execute care plans.
  4. Provide education, health promotion, and coaching to support self-management.
  5. Support members and families with individual assistance.
  6. Coordinate discharge planning and care transition following ER, inpatient, or SNF admissions.
  7. Advocate for members to ensure their needs and choices are represented.
  8. Research and refer members to community resources, following up to confirm receipt of services.
  9. Engage proactively with members to manage care, providing education and support as per the care plan.
Required Qualifications
  • Bachelor’s Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences
  • Certificated Medical Assistant with associate degree, Licensed Practical Nurse, or Licensed Marriage and Family Therapist, Mental Health Counselor, or Agency Affiliated Counselor
  • Master’s Degree in relevant fields
  • Must become a certified Health Home Care Coordinator within six weeks of hire
  • 3+ years of relevant clinical experience
  • 1+ years of experience with MS Office and Teams
  • Reliable transportation and ability to travel up to 75% within King County
  • Reside in or near Seattle, WA
Preferred Qualifications
  • CCM certification
  • Experience in community case management for complex needs
  • Background in Managed Care
  • Team-based care experience
  • Bilingual abilities
Physical Requirements
  • Ability to transition between office and field locations multiple times daily
  • Navigate various terrains for visits
  • Transport equipment as needed
  • Remain stationary for computer work
  • Adhere to telecommuting policies

The hourly pay ranges from $23.70 to $46.35, based on experience, education, and other factors. Benefits include comprehensive packages, incentives, stock options, and 401k.
