The Health Home Field Care Coordinator will be the primary case manager for a panel of high-cost, high-risk Medicare-Medicaid enrollees, focusing on intensive care coordination services to improve health care outcomes and cost savings. Care Coordinators will integrate care across multiple delivery systems, working to reduce gaps in services and increase coordination among all service providers including medical, behavioral health, long-term services and supports, and social services. This is achieved through principles of patient activation and engagement to set health action goals and increase self-management skills for optimal physical and cognitive health.

If you are located preferably in King County, WA, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities
  1. Engage members, family, and caregivers face-to-face to complete comprehensive health needs assessments, including medical, behavioral, functional, cultural, and socioeconomic needs.
  2. Develop and implement person-centered Health Action Plans to manage chronic conditions, promote health and wellness, address social determinants of health, medication management, and member safety, aligned with evidence-based guidelines.
  3. Partner and collaborate with internal care teams, providers, and community resources to implement care plans.
  4. Provide education, health promotion, and coaching to support member self-management and lifestyle changes.
  5. Offer individual and family support for members.
  6. Support proactive discharge planning and coordinate Care Transition following ER visits, inpatient stays, or SNF admissions.
  7. Advocate for members and families to ensure their needs and choices are fully supported by the healthcare team.
  8. Research, locate, and refer members to community resources to address specific care or socioeconomic needs, and follow up to confirm receipt of services.
  9. Proactively engage members to manage their care, providing education and coaching as outlined in the care plan.

Required Qualifications
  • Must meet one of the following:
    • Bachelor’s Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences
    • Certificated Medical Assistant with an associate degree
    • Licensed Practical Nurse
    • Licensed Marriage and Family Therapist, Licensed Mental Health Counselor, or Licensed Agency Affiliated Counselor
    • Master’s Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences
  • Must become a state-certified Health Home Care Coordinator within six weeks of hire.
  • 3+ years of relevant, including clinical, experience.
  • 1+ years of experience with MS Office (Word, Excel, Outlook, Teams).
  • Reliable transportation and ability to travel up to 75% within King County.
  • Reside in or near Seattle, WA – King County.
Preferred Qualifications
  • CCM certification.
  • 1+ years of community case management experience for individuals with complex needs.
  • Background in Managed Care.
  • Experience in team-based care.
  • Bilingual abilities.
Physical Requirements
  • Ability to transition between office and field locations multiple times daily.
  • Navigate various terrains for visits.
  • Transport equipment for visits.
  • Remain stationary for long periods for computer work.
  • Adhere to remote work policies.

The hourly rate ranges from $23.70 to $46.35, based on full-time employment and various factors including experience, education, and certifications.
