Coordinates advocates and manages groups of patients during the continuum of care delivery. Provides evidenced based standard of care to patients via the therapeutic use of self the scientific process and the environment instrumentation in conjunction with other health care team members. Collaborates with physician-led multidisciplinary care team. Ensures appropriate use of clinical resources for optimal patient clinical outcomes.
