Job summary

Broward Health's Children's Diagnostic & Treatment Center is seeking a social work professional to provide ongoing case management for children and youth with special health care needs within a medical home model.

You will coordinate appointments with specialists, schedule medical tests and exams, and assist families with financial and social assistance while ensuring participation in the continuous planning of patient care.

Qualifications

• Bachelor's degree in Social Work
• At least 1 year of related experience
• Experience in case management or care coordination is a plus
