Job summary

Cumberland County Guidance Center is seeking a Full-Time Care Coordinator to join the Project Redirect team in southern New Jersey. You will connect participants with behavioral health services, housing assistance, and community resources while working with courts and providers to promote engagement and outcomes.

Qualifying applicants have a bachelor’s in a mental health field, at least two years of related experience, and the ability to obtain jail clearance.

Qualifications

• Bachelor's Degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or another mental health-related field from an accredited institution
• Minimum two (2) years of related professional experience
• Ability to obtain jail clearance.
• Ability to build collaborative relationships with community agencies, service providers, and justice system partners

Responsibilities

• Coordinate services between behavioral health providers, criminal justice agencies, and program participants
• Assist individuals in accessing mental health treatment, community resources, and support services
• Conduct assertive community outreach and engagement activities
• Monitor treatment participation and support adherence to program requirements
• Collaborate with clinicians, courts, and community partners to support successful program outcomes
• Promote access to housing, benefits, healthcare, employment, and other community resources
• Assist with data collection, reporting, and program documentation requirements
• Attend Municipal and Superior Court proceedings to identify and engage potential Project Redirect participants

Cumberland County Guidance Center is seeking a Full-Time Care Coordinator to join our Project Redirect team.

This opportunity is ideal for professionals with experience in case management, behavioral health, community outreach, social services, reentry services, criminal justice programs, or human services who are passionate about helping individuals connect with treatment, reduce barriers to care, and achieve lasting success in their communities.

As a Care Coordinator, you'll serve as a liaison between behavioral health providers, criminal justice partners, and program participants while helping individuals engage in treatment services and successfully complete Project Redirect.

What You’ll Do

As a Care Coordinator, you'll work directly with individuals participating in Project Redirect, helping connect them to behavioral health services, community resources, and support systems that promote treatment engagement and successful community integration.

Key Responsibilities

• Coordinate services between behavioral health providers, criminal justice agencies, and program participants
• Assist individuals in accessing mental health treatment, community resources, and support services
• Conduct assertive community outreach and engagement activities
• Monitor treatment participation and support adherence to program requirements
• Collaborate with clinicians, courts, and community partners to support successful program outcomes
• Promote access to housing, benefits, healthcare, employment, and other community resources
• Assist with data collection, reporting, and program documentation requirements

Required Qualifications

Education & Experience

• Bachelor's Degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or another mental health-related field from an accredited institution

• Possessionof a valid New Jersey driver's license with a clean five-year driving record that meets organizational standards and qualifies for insurance coverage.
• Ability to work a flexible 40-hour schedule

Preferred Qualifications

• Experience working in behavioral health, mental health services, case management, or community outreach
• Experience collaborating with criminal justice agencies, diversion programs, or reentry services
• Knowledge of resources available to individuals living with mental illness and those with justice-system involvement
• Experience supporting individuals transitioning back into the community
• Spanish-speaking ability
• Experience working with multidisciplinary treatment teams
