Job summary

Enloe Health in Northern California is seeking a Care Coordinator to manage the care and services for assigned patients across an episode of illness or treatment, collaborating with patients, families, physicians, the patient care team, and payors.

This role uses advanced clinical nursing skills to facilitate assessment, planning, intervention and evaluation, promote resource utilization, and lead the care team toward optimal, cost-effective outcomes.

Qualifications

• Two years of acute care clinical experience or a Master's in Nursing with a Case Management emphasis.
• Bachelor’s degree in nursing is desirable.
• Current licensure with the California Board of Registered Nursing plus CPR recognition.

Responsibilities

• Coordinate care and services for assigned patients across an episode of illness or treatment.
• Collaborate with patients, families, physicians, care team and payors to ensure optimal outcomes.
• Advocate for patients and maintain communication among providers throughout the episode of care.
