Job summary

Main Line Health is seeking a Care Coordinator MSW (Social Worker) to lead cross-continuum care coordination and psychosocial assessment for patients. You will partner with physicians, clinical teams, and community resources to enhance patient care, safety, and population health in a hospital setting.

The role requires a Master’s in Social Work from a CSWE-accredited program and a strong background in a physician’s office, clinic, hospital, or EMS environment, with licensed social worker

Qualifications

• At least 5 years of recent clinical social work experience in hospital, physician's office, clinic, or EMS settings.
• Master’s Degree in Social Work from CSWE-accredited program.
• Licensed Social Worker strongly preferred.

Responsibilities

• Coordinate cross-continuum care and coordinate patient care across settings.
• Conduct psychosocial assessments and implement interventions.
• Collaborate with physicians, peers, and community partners to improve patient care.
