Job summary

Main Line Health is seeking a Care Coordinator MSW at Lankenau Medical Center to provide cross continuum care coordination with physicians and care teams. The role includes psychosocial assessments, interventions, and developing patient-centered care plans in a hospital setting.

Position: Care Coordinator MSW

Shift: Monday-Friday; Day Shift plus weekend rotation

++Experience:++

1. At least 5 years of recent clinical experience in a physician's office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
2. Leadership experience a plus.

Master's Degree in Social Work from CSWE (Council on Social Work Education) accredited program.
