Job summary

Endeavor Health is seeking a Care Coordinator-MSW in Arlington Heights to collaborate with continuum care managers and a multidisciplinary team to support patients with chronic illness and multiple comorbidities.

The role requires an MSW with IL licensure (LSW/LCSW preferred) and 1–2 years of acute care experience. You will coordinate care, educate patients, and support safe transitions to improve outcomes.

Qualifications

• MSW degree and IL social work licensure required.
• 1–2 years of social work experience in acute care preferred.

Responsibilities

• Collaborate with multidisciplinary teams to support patients with chronic illness.
• Coordinate care across departments and community resources.
• Apply social work methods to assess, educate, and coordinate care.
• Facilitate advance care planning and goals of care discussions.
• Ensure safe transitions of care and appropriate utilization.

Position Highlights:

• Position: Care Coordinator-MSW
• Location: Arlington Heights
• Full Time: 40 hours per week
• Hours: Monday-Friday, 8:00a-4:30p, rotating every 9th weekend, and 11 winter and 1 summer holiday coverage required

What you will do:

Collaborates with Continuum Care Managers, Case Managers, Care Navigators, Physicians, Nursing, PCP & Collaborates with Continuum Care Managers, Case Managers, Care Navigators, Physicians, Nursing, PCP & multidisciplinary team to support patient populations with chronic illness and multiple co-morbidities.

Adheres to established care management standards that incorporate evidence based protocols and cost effective, patient focused, targeted interventions.

Provides coordination between multiple departments, multi-disciplinary team, medical clinics, and community outreach to gain knowledge of patient, assure patient safety, assure smooth transitions of care, and manage utilization and the total cost of care.

Applies social work methods to skilled assessment, education, collaboration, and coordination of healthcare and community resources to achieve the following: assist patients to gain self-efficacy/management skills, achieve optimal functional health status, and improve quality of life. Assists patient/families, care team, and systems to achieve high quality outcomes through face to face and telephonic interactions. -

Provides support services to Endeavor patients and all of its value-based contract members including but not limited to care coordination and counseling.

Provides intervention in child abuse/neglect, domestic violence, guardianship (temporary/ permanent), mental health placement, substance abuse, advance directives, adult/elderly/disabled abuse and neglect, and sexual assault.

Ensures safe care for patients while adhering to policies, procedures, and standards, within budgetary specifications, including time management, supply management, productivity, and accuracy of practice.

Initiates and facilitates Advance Care Planning and goals of care discussions with patient and family as appropriate and also as programmatically indicated through patient risk stratification program(s).

Demonstrates the knowledge and skills necessary to assess and care for the assigned age groups(s) of patients. Implements assessment and care based on the developmental stages of the patient. Involves family/guardian in the plan of care and conducts patient and family education, as appropriate, considering the developmental needs of the patient and the impact of social/behavior issues on the illness and recovery process.

What you will need:

Master's Degree in Social Work required. Illinois licensure as LSW; 1 to 2 years of social work experience in an acute care setting preferred

Minimum Education Level

Master's Degree in Social Work required from an accredited school of Social Work.

Minimum Licensure Requirements

Illinois licensure as LSW or LCSW; LCSW preferred.

Minimum Certification Requirements

Licensed Social Worker (LSW)

Minimum Work Experience

1 to 2 years of social work experience in an acute care setting preferred.

Unique or Preferred Skills:

• Able to communicate effectively and work collaboratively with a range of stakeholders and team members.
• Able to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multidisciplinary team, and community agencies to achieve desired outcomes.
• Act as a subject matter expert.
• Teamwork-active participation with performance improvement and other approaches. Collaborates with the team members to develop effective work processes that my lead too improvement of work

Hourly Pay Range:

$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.
