Job summary

Meridian Healthcare in Gainesville, FL is seeking a Care Coordinator to assess needs, coordinate services, and advocate for adults with opioid use disorders receiving MAT. This role focuses on ensuring coordinated, comprehensive care.

You will perform assessments, develop service plans, coordinate with providers, monitor progress, and maintain documentation. A bachelor’s degree and at least 1 year of care management experience are required, plus a Florida driver’s license.

Qualifications

• Bachelor’s degree in healthcare, psychology, social work, or related field.
• 1+ year of care management experience (SUD, SMI, or similar populations).
• Valid Florida driver’s license with clean record.

About the Role

As a Care Coordinator, you’ll independently assess needs, coordinate services, and advocate for adults with opioid use disorders receiving Medication Assisted Treatment (MAT). You’ll help ensure clients receive comprehensive, coordinated care while maintaining continuity of services.
