Job summary

Physicians’ Primary Care of Southwest Florida in Fort Myers, FL is seeking a Care Coordinator for our Family Practice Division. This full-time role sits in our Administration building and focuses on coordinating care and improving patient outcomes.

The incumbent will develop chronic disease management programs, review schedules and charts, and serve as a key communication link between patients and the medical team, while upholding HIPAA standards and patient confidentiality.

Qualifications

• Minimum of three (3) years of clinical experience.
• Great attention to detail and the ability to record information accurately.
• Ability to develop and maintain a good rapport and a cooperative working relationship with providers, staff, and patients.
• Prior experience in care management, quality improvement, medical records reviews, disease/case management preferred.
• Graduate of an accredited school of nursing with a valid and active Florida PN License or RN License.

Responsibilities

• Develop, implement, and carry out chronic disease management programs for patients (e.g., diabetes, CAD, COPD, heart failure).
• Review provider schedules and patient charts; coordinate care for visits and future needs using Care Management criteria.
• Develop and coordinate patient outreach programs.
• Provide an effective communication link between patients and medical staff.
• Maintain confidentiality and adhere to HIPAA guidelines and regulations.

Physicians’ Primary Care of Southwest Florida is a premier physician owned and managed primary care practice with locations in Cape Coral, Estero, Fort Myers, and Lehigh Acres.

We are currently seeking Care Coordinator for our Family Practice Division in Fort Myers, FL.

This position is conveniently located in our Administration building located off of College Parkway.

This is a full-time position. The schedule is flexible, Mon-Fri, Days.

Sample of Duties:

• Work with medical staff to develop, implement, and carry out programs in chronic disease management for patients with such problems as diabetes, coronary artery disease, COPD, and congestive heart failure.
• Review provider schedules and individual patient charts and assist the care team in coordinating care for visits and for future healthcare needs using Care Management criteria.
• Develop and coordinate patient outreach programs.
• Provide an effective communication link between patients and medical staff.
• Maintain the strictest confidentiality and adhere to all HIPAA guidelines and regulations

Position Requirements:
