Job summary

Presbyterian Healthcare Services seeks a Care Coordinator IV to lead a collaborative care model across medical, behavioral, social and community services. You will coordinate assessments, develop and monitor care plans, and guide interventions that optimize outcomes for complex patients.

The role emphasizes teamwork, cost-effective care delivery, and partnerships with caregivers and providers to support patients through chronic conditions and transitions of care.

Qualifications

  • Masters degree with 4+ years of experience
  • Bachelor's degree with 8+ years of experience
  • Associates degree with 9+ years of experience may substitute for other education/experience requirements

Responsibilities

  • Facilitate a team approach with the Interdisciplinary Care Plan team to ensure cost effective, quality care across the continuum.
  • Coordinate care for members with chronic/complex conditions using assessment, care planning, implementation, monitoring and evaluation.
  • Provide strategies to improve health outcomes and quality of life for members and families.
  • Collaborate with care team, caregivers, physicians, and community resources to address medical, behavioral and social needs

The Care Coordinator facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost effective delivery of quality care and services across the continuum. Collaborates with the interdisciplinary care plan team which may include member, caregivers, member s legal representative, physician, care providers, and ancillary support services to address care issues, specific member needs and disease processes whether, medical, behavioral, social, community based or long term care services. Coordinates care of individual clients with application to identified populations using assessment, care planning, implementations, coordination, monitoring and evaluation for cost effective and quality outcomes.Some key responsibilities include:Facilitates a team approach, including the Interdisciplinary Care Plan team, to ensure appropriate interventions, cost effective delivery of quality care and services across thecontinuum. Collaborates with the Interdisciplinary Care Plan Team which may include member, caregivers, members legal representative, physician, care providers, andancillary support services to address care issues, specific member needs and disease processes whether, medical, behavioral, social, community based or long term careservices.Conducts in depth health risk assessment and/or comprehensive needs assessment which include, but not limited to psycho-social, physical, medical, behavioral,environmental, and financial parameters.Provides care coordination to members with chronic or complex conditions which require intensive interventions and oversight include multiple, clinical, social and communityresources.
