Position Purpose

Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, maintaining continuity of care and member satisfaction. Interacts with members via outreach calls or home visits, documenting care plans and activities.

Remote position, but must be in Florida for training. Must reside in Florida.

Responsibilities
  1. Provides outreach to members via phone or home visits to discuss care plans, next steps, resources, and ongoing education.
  2. Coordinates care activities based on care plans, working with healthcare providers, community partners, and members/caregivers.
  3. Serves as support for member and provider inquiries related to care plans.
  4. Communicates with care managers and practitioners to facilitate member services and ensure continuity of care.
  5. Supports service assessments and documents member care needs.
  6. Maintains member records in compliance with regulations and shares information with providers as needed.
  7. Follows standards and policies aligned with contractual and regulatory requirements.
  8. Identifies needs and makes referrals to Care Managers, community organizations, and Disease Managers.
  9. Provides education on benefits and available resources.
  10. Performs other duties as assigned and complies with all policies and standards.
Qualifications
  • High School diploma or GED required.
  • 1–2 years of related experience required.
Additional Information
  • For Florida - All interactions are telephonic.

This is a remote position, but training requires presence in Florida.

Pay Range: $17.50 - $27.50 per hour.
