Centene Corporation in Fort Smith, AR is seeking a Care Coordinator to support care management activities and ensure continuity of services for members. Responsibilities include member outreach, care plan coordination with providers, and documentation in compliance with regulations.

Position Purpose: Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care/member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care/services of activities.
Key Details: Candidates must reside in or near Sebastian or Crawford County, Arkansas, and be able to travel throughout the assigned service area to conduct member visits and coordinate care.
Education/Experience: Requires a High School diploma or GED.

Pay Range: $17.84 - $28.02 per hour
