Job summary

Talentify is seeking a Care Coordinator in Camden, NJ to develop care plans for referred clients and link them to supportive services. The role provides overall support to clients facing barriers to access and retention in care, helping with appointments, resources, and program applications.

Qualifications

• Bachelor's degree is required or preferred.
• Three years of experience in the healthcare/social services field is preferred.
• Strong organizational and time-management skills are essential.

Responsibilities

• Complete Care Plans on all assigned clients based on approved agency assessment tools.
• Assist clients with understanding care plans and motivate active participation in their health.
• Assist clients with scheduling medical appointments and follow-up to ensure attendance.
• Assist clients with arranging transportation to appointments and accompany them as needed.
• Maintain timely, accurate documentation in accordance with agency requirements.
• Attend team meetings and coordinate care with providers and office staff.
